Good Morning Mr. Swift, I'd like to request your assistance in receiving full Short Term Disability Benefits for my claim, which was initially approved on 04/28/2016. My claim contact is Michelle  and she reports data to Shelby. My concern is that I have not received 70% of my wages as necessary; I and my doctor  who removed me from work on 04/21/2016 given great depression, stress, and other emotional concerns, not to mention the two surgeries that I'll undergo on 06/13/2016. To date my physician and I have expediently responded and met every requirement of Michelle Class and Shelby Jetton; to date I've only been approved for payment through 05/22/2016, thus payment receipt are behind as it's 06/03/2016; I want to do everything necessary for accurate and timely payment disbursements. I have consistently asked for the requirements which would allow me to proactively return all necessary documents prior to The Hartford deadlines; unfortunately Michelle and consistently evasive and also repetitive in stating "The medical data must suffice." I feel as though I'm being spoken down to and not receiving quality assistance. What more can I provide than what my doctor has provided, via fax, on (3) or more occasions. My first payment disbursement was 70% ($823.54 on 05/13/16:) the second was 42.4% ($349.00 received on 05/31/16) which just enough to pay two office visits that totaled $350.00. I am quite frustrated and I'd like resolve this concern immediately; this process is causing great frustration for me and intensifying my depression.
