Sandra sent a message to Stephen Hemsley – Chief Executive Officer of UnitedHealth Group Inc. email address that said:
Hi Mr. Hemsley,
I have been attempting to get my mother's medical claims history from your company since June of this year to no avail.
I was told that in order to get a detailed history of her claims information that includes diagnosis, procedures done, and medications that I needed to fax in a letters testamentary showing me as her executrix which I did on June 2, 2016. I was told that I should begin to receive that information within 10 days. I received the explanations of benefits.
I called back and explained that the explanation of benefits, while helpful, is not what I asked for.
Here are the dates that I have talked with the company via telephone: 06/02/2016, 06/14/2016, 08/04/2016, 08/24/2016, and 09/08/2016.
On September 11, 2016, I sent another copy of the letter testamentary along with the company hipaa form requesting this information via FedEx to your Utah office which was received on Thursday, September 15, 2016.
To shorten and summarized this long incredible story, to date, I have received 3 copies of explanation of benefits, several copies of a few prescriptions then nothing.
I have done as I was asked to do. Please help me with this request for my mother's medical claims history.
