With a copy of my complete pension file including but not limited to : a copy of my complete work history used to evaluate my entitlement to benefits and a copy of any communications and evaluations used to determine my entitlement to benefits. Also; A copy of my latest individual benefits Statement. In particular, I am requesting a benefit calculation sheet that explains how my pension benefit is currently being calculated.
