Dear Mr. Cordani :
Please be advised that I am contacting you regarding a third denial of a short term disability claim that has been overseen by Mr. Eric Fantaske, and Ms Katie Huerbin one of your appeal managers in the Pittsburgh service center. I spoke with Ms. Huerbin earlier this morning advising her that I disagreed with the denial of this claim . Ms. Huerbin was attitudinal and advised that the only other option for me was legal action Additionally, I have repeatedly requested the legal definition of functional impairment My incident number 4142835. I respectfully request that you review the facts of this claim particularly documentation provided by Dr Peter Baicco dated January 19,2017
By their actions, in my opinion, your medical professionals have wrongfully concluded that I was able to work effective 12/23/2016 even although I had a viral infection necessitating the postponement of an endoscopy, I was in extreme pain and continue to be treated for a cardiac condition that is exacerbated by stress. Mr. Cordani, I ask how you would feel if you were in my position after paying premiums to Cigna in good faith
with the expectation that I would be covered when I was unable to work due to a series of health maladies.
