I was approved by Cigna for LTD on 10-22-20 for the period of 8-17 thru 10-22 of which I have received the payment. During 8-17 to 9-25 without notice or a call from Capital One HR, I was paid PTO that has to be paid back. Now I have received a letter stating the leave mngt team records show I am on a unpaid leave and directed Anthem BCBS to bill me $690 for 3 months.
