On May 31, 2019, I mailed you the âProof of Lossâ form, police report and copies of information regarding my stolen cell phone. I visited the T-Mobile store on Gallatin Road in Nashville, TN, to report my stolen cell phone. The T-Mobile supervisor (Lindsey) talked to your company and talked through the process of filing the stolen phone claim. Your company said the form information was complete.
Then I paid the deductible fee with my visa card. After weeks of not receiving the
new cell phone, I called Assurant to inquire about the delay of the delivery of my
cell phone and their reply was that they did not remember the telephone conversation and that I had to fill out the phone online or they could mail me
the form. My reply was to have the form mailed to me. I received the form and
immediately mailed the form to Assurant on May 31, 2019. Yesterday, I returned
to the T-Mobile store and a sales representative called your company and the
young lady said they had not received the âSworn Proof of Loss Form.â
This situation is very frustrating, since I have returned the form and paid the deductible fee. Please Help!
