Joyce sent a message to Stephen H. Rusckowski – President and Chief Executive Officer of Quest Diagnostics – Email Address that said:
Dear Mr. Ruskowski,
I spoke with Jodi (Quest Diagnostics employee) and she asked me to e-mail my completed Request for Release of Pathology Materials Form. I informed her that I submitted a complaint to you in regards to Quest Diagnostics' non-compliance.
The Release Form has an option for the patient to complete the form as the Requestor. It is incomprehensible that Quest Diagnostics is refusing to send my cervical slide(s) to the location specified on the completed form.
Please assist me with the processing of this urgent matter. Thank you,
