Dear Mr. Swedish,
I am writing to you today to ask for your help in processing a denied claim. I have called and written to Anthem numerous times with no success.
I have a rare eye condition that requires me to receive monthly injections in my eye to prevent loosing my vision.
Anthem has denied my claim on 9/12/2016 # 2016265490956, but processed this same treatment on 11/12/2016 # 2016301490157. My Doctor Ivana K. Kim, M.D. Director, Macular Degeneration Unit At Massachusetts Eye and Ear has called repeatedly to try to help get my treatments approved, and to help process this claim.
I would appreciate any help you can provide. I am a teacher and administrator in a school in Boston, and my vision is imperative to my job.
