Good Afternoon Mr. Carmichael,
I am sorry to bother you with a concern of a stop fee on a check. It was brought to my attention, there is a fee of $33.00 and it not non-refundable. I am on a fix income. I sent a check number 134 to Envision RxPlus. They notify me that they have not received as of yet. I sent the check in their pre-labled enveloped.
When I was forcefully had to join them, I had the impression that they would cover my diabetic supplies and well as my medication. I have even asked several times to be certain that they would cover all my needs. I was wrong in believing Envision RxPlus. Now, I received a letter indicating of disenrollment and I will be charged(?) if payment is not in by July 1st of 2016. I have made contact with their accounting department and still no success. They did not received or was offered to cancel the check and make a payment over the phone. I can not afford to submit a double payment.
I beckon you to uplift this fee or lower the fee in stopping the check in being submitted. I am handicap and live on a fix income. I also have my benefit direct deposit to your bank for long awhile now.
I need of your assistance. If I may...
