Hello Amos

My name is Simone, I’ve been with AFLAC for about ten(10) years. I filed a claim with AFLAC in August 2016. First the claim was denied, the I sent my DC Government representative(Jean-Claude Fresnel, Jr.)an email and he said he was going to look into my claim. Mr. Jean-Claude Fresnel, stated that the claim was denied because the hospital I was in. I’ve sent Mr. Jean –Claude Fresnel and email requesting his super name and number. Jean-Claude Fresnel responded to my email, his manager is Kari Ship(AFLAC District Administrator. I’ve tried to reach Ms., Kari several time and she has never returned my calls. Mr. Jean-Claude, has completely stop all communication with me. I’ve never spoken to Mr. Jean-Claude and he’s stop responding to my email.

Then, I reach out to customer service and requested and supervisor. The supervisor that has been assisting me is Clydia. Mrs. Young requested me to speak to my doctor’s and request letter from my doctor’s to let AFLAC know the medication I was on was prescribe for my current health issues. Mrs. Young read in the fine print to my policy for my hospital policy. If the medication I was on is the reason I went into the hospital or the cause of me being hospitalize. The claim can be granted, the letter were giving to the auditors and the still denied my claim.

I’m not sure who the auditors are but there is no way they could have possibly read the fine print to my policy. I have attached to letters from my Internal Medicine Doctor and my surgeon from Kaiser Permanente. I writing you because I need your assistance with my claim. I been with DC Government for twenty-seven(27) years and AFLAC for ten(10) and I have never received such poor customer service in my life. I have an illness that I didn’t do to myself, and treatment I’ve received is so unfair. I have discuss this with my Human Resource Office, and would like to know the out-come. Thank you so much for taking time out of your busy day, to read my letter.

P.S.
My pain management doctor is providing a letter and my other surgeaon.

Simone
