Letter to Christopher J. Swift (Hartford CEO) 07/29/16 10:31am

Good Morning Mr. Swift,
I'd like to request your assistance in receiving full Long Term Disability Benefits for my claim my Short Term Disability was initially approved on 04/28/2016. My Long Term Disability claim contact is Dianne Matheny. My concern is that I have not received consistently received 70% of my wages as necessary; today I received an electronic deposit of $250.30 (which is approximately 33.4% of what the 70% benefit.) I have proactively inquired of Dianne Matheny the necessary documents I am responsible for submitting; both I and my doctors (Dr. Debora L. Ferguson MD – PSYCHIATRY, Dr. Michael Cotant, and Dr. Robert Ferguson) have expediently submitted all of the requested documents required by Dianne Matheny; at this point it’s over kill. Unfortunately, Dianne has mailed me erroneous information and continues to add other documents over and above what was initially requested. My claim can never be approved if this cycle continues. Dr. Debora Ferguson removed me from work on 04/21/2016 given great depression, stress, and other emotional concerns, not to mention the two surgeries that I'll undergo on 06/13/2016. I want to do everything necessary for accurate and timely payment disbursements. I have consistently asked for the requirements which would allow me to proactively return all necessary documents prior to The Hartford deadlines; unfortunately Dianne has consistently evaded and or added additional medical documents required. I’m not sure whether the goal of Hartford claim representatives is to frustrate individuals, but this has been my experience. What more can I provide than what my doctors and I, have provided, via email and fax on multiple occasions. To recap, I received 42.4% ($349.00 received on 05/31/16) which just enough to pay two office visits that totaled $350.00. I ask for your assistance to resolve this concern immediately; this process is not only causing great frustration for me and intensifying my depression. I have not had an opportunity to properly rest and heal from my surgeries, and other concerns, given the persistent banter that I’ve been subjected to. Sir, any assistance that you can offer would be greatly appreciated. I can be reached
