@libertymutual – David Long – Dear Mr Long On Friday October 25, 2019 my attorney Mr Stephen F Grover received

Christine Quinn sent a message to David Long that said:

Dear Mr Long
On Friday October 25, 2019 my attorney Mr Stephen F Grover received an email from your company dated 10/03/19
It was addressed to me and indicated
Action is Required" with a First B-Notice requiring an updated w-9 form. This is likely in response to a worker's comp claim settled last year with CP Franchising LLC
The social security number on the form letter received is incorrect. I have the typed w-9 I have been using for five years. After lengthy discussions with SSA and IRS I have been told the W-9 needs to be sent to Liberty Mutual.
The form letter states " You may send forms via fax to this number: " No number is listed. I have been on hold with Liberty today from 4:03 PM until now at 5:58 PM. Eight times I have been disconnected (most recently by Reese in Workers Comp dept)

All I need is assistance in sending the correct original W9 to Liberty Mutual. The form letter states if not resolved by Nov 1 I am subject to back-up with holding. I am trying my best to forward the correct form to Liberty and still have no idea where to send it. Each department I speak with transfers me someplace esle.
Can you ask someone to help me please?
