I went to quest center (80 Mill River St, Stamford, CT 06902) Oct 10, 2019, around 10 am for a drug screening test send by Montefiore, authorization Id is: 38263661, and I gave a urine sample as per instruction by the center employee, like don't flash commode, etc., she told my result will be sent to my employer within 24-48 hrs.
Attached following survey request I got from that visit.
After one month my employer told me upon my inquiry that your office reported them âno showâ. How itâs possible. Is your center use my urine to clear a drug addict?
